Support coordination plays a crucial role in assisting participants in the National Disability Insurance Scheme (NDIS) to navigate their plans effectively and achieve their goals.
Here's how it helps:
1. Connecting to NDIS and other supports: Support coordinators help participants in understanding the NDIS and connecting them to various supports available within the scheme as well as other community-based supports.
2. Organise supports and services: They work closely with participants to identify their needs, preferences, and goals, and then assist in finding and arranging supports and services that align with these factors while staying within the budget outlined in their plan.
3. Monitoring plan budgets and support effectiveness: Support coordinators keep track of the participant's plan budget and the effectiveness of the supports and services being utilised. They ensure that the participant's budget is being utilised optimally and make adjustments if necessary.
4. Building capacity and capability: Support coordinators empower participants by providing them with the knowledge and skills to understand their plan, navigate the NDIS system, and make informed decisions about their supports. They aim to enhance the participant's ability to self-manage their plan and exercise choice and control over their supports.
The assistance provided by support coordinators is tailored to the individual participant's goals, needs, and circumstances, as well as what is funded in their NDIS plan. There are three levels of support coordination that can be included in a participant's plan, which vary based on the participant's level of need and complexity of their circumstances.
These levels are typically described as:
1. Level 1 - Support Connection: This level provides assistance to help participants understand and implement their plan. It's often suitable for participants with less complex needs who require minimal ongoing support coordination.
2. Level 2 - Coordination of Supports: This level involves more active support coordination to help participants with moderate needs. It includes assistance in identifying, arranging, and coordinating supports and services, as well as monitoring their effectiveness.
3. Level 3 - Specialist Support Coordination: This level is designed for participants with more complex needs or situations requiring specialist expertise. It involves intensive support coordination, including crisis management, coordination with multiple service providers, and building capacity to manage supports independently over time.
These levels of support coordination ensure that participants receive the level of assistance that best matches their individual circumstances and helps them achieve their goals within the NDIS framework, managing and coordinating your supports efficiently.
